Appeal Request 
The graduate student academic appeal procedure is designed to offer a means to resolve disagreements related to interpretation of University or department policies for students enrolled in graduate courses in the University.

Application for certificate 
Use this form to apply for a graduate certificate. MBA students must apply for certificate separately from degree.

Application for degree 
Use this form to apply to graduate. Once you have completed the form, pay your $75 fee for a master's program or your $100 fee for a doctorate program at the Student Accounts Office. There is an additional $10 fee if you miss the deadline (check Bulletin for deadlines). After paying the graduation fee, submit the form to the Graduate School to initiate an audit. You will receive a letter informing you of the courses you need to complete in order to graduate.

Change of Program within Department 
This form is used to change your specialty within a department and may not be used to apply to a different department or college. 

Independent Study Form 
Complete this form in its entirety with your instructor. Specify the plan for completing the study, indicating if it will follow the syllabus of a standard course.

Intent to Submit and Present Thesis Form 
This form is to be used to submit thesis for binding.

Leave of Absence
Typically, master’s students who stop out for less than 2 years are not required to apply for a leave of absence. A student may wish to submit the Leave of Absence form as a record of intention to return to the program. Some programs (particularly nursing) require notification of leave of absence because clinical assignments are limited. 

MIGS Form (Graduate Guest Pass) 
The Michigan Intercollegiate Graduate Studies form is used with permission of students' advisor and Dean of the Graduate School to take a course as a guest graduate student at another Michigan institution.
Recommendation Form
This form may be used for the recommendation from a professional associate or supervisor. Provide the form to your reference and ask them to send it in a letterhead envelope from their business or a stamped envelope that you provide. They should sign over the seal to indicate it was not viewed by you.
Statement of Purpose Form 
This form is for applicants to the MALS, MBA, and nursing programs.
